Mini/U6 families: The Monday/Wednesday group is included in the 7:30 AM and 10:15 AM sessions. Check with your coach about which session to attend.

Three easy steps
- Find your favorites. Browse the sample sheet and the complete package descriptions and prices on the order form.
- Fill, save, and print. Enter your player’s information, item codes, quantities, prices, and total in the fillable PDF. Save a copy and print it. If your browser doesn’t support the fields, download the PDF and open it in Adobe Acrobat Reader.
- Bring it to picture day. Give the printed order form to your child to bring, with payment arranged according to the photographer’s instructions.
The PDF is an order form to complete and print; it does not submit an order or process payment online.
A keepsake for every kind of fan
- Rookie — $20: a simple starting package.
- All Star — $35: a classic photo package.
- MVP Personalized — $50: personalized with your player’s first name and year.
- Modern Styles — $75: Clean Look, X-Factor, or Smoky, including individual and team downloads.
- Buddy / Family — from $25: photos with two or more people. Tell the photographer; these are taken immediately after the individual photo.

Every player who places an order receives a bonus 8×10 and button, limited to one set per player.
Payment & helpful details
Avery Wham Photography accepts cash, Venmo, Zelle, or check. Make checks payable to Avery Wham Photography. Refer to the order form for the photographer’s payment details.
Ordering trader cards or an X-Factor design? Complete the player statistics requested on the form.
About digital downloads: The photographer delivers downloads through a temporary password-protected gallery that can also be viewed by other parents who order downloads. Ordering a download signifies consent to that delivery method.
